Child and Adolescent Behavior Screening


Attention-Deficits/Hyperactivity



Please rate your child and ask one other person who knows your child well to rate your child using the following scale:

Does your child have difficulty sitting still?
Is your child’s behavior marked by excessive activity?
Is your child impatient?
Does your child frequently do things without thinking?
Does your child cause or threaten harm to others, self, animals, or objects?
Does your child lose his/her temper?
Does your child argue or refuse to comply with direction?
Does your child annoy others?
Does your child blame others for his or her own misbehavior?
Is your child easily annoyed?
Is your child angry and resentful?
Is your child spiteful and vindictive?
Does your child seem depressed? (See Depression Screening.)
Does your child seem anxious? (See Anxiety Screening.)
Have you noticed changes in usual patterns of behavior? (See Addictions Screening.)

Consider a professional assessment if:
Your child gets physically aggressive or engages in dangerous behavior.
You are concerned about the checked symptoms.
You checked “3’s” and “4’s” and have noticed this behavior before your child turned seven and you have noticed it for at least six months.
These behavior occur in at least two settings, i. e. school and home.
These behavior interfere with daily tasks.

Are Kids Today Harder to Raise?
